Coronary artery fistulae are the uncommon abnormality which can be congenital or acquired.
Occurrence of clinical symptoms depends on the rate of hemodynamical dysfunction. Sudden
death often occurs as the first symptom of this anomaly. Because of variety of signs and lack of
established guidelines in such clinical situations, we present the case of 49-year-old female
with coronary artery fistulae between left anterior descending coronary artery and main pulmonary
artery and between right coronary artery and main pulmonary artery. The patient was
admitted to hospital due to unstable angina pectoris. The diagnosis was established during
coronary arteriography. The patient did not give her consent to percutaneous closure of the
fistulae. Intensive treatment of hypertension and hyperlipidaemia as well as change in lifestyle
